ACVO CREDENTIALS COMMITTEE INFORMATION

TWO STAGE CREDENTIALING PROCESS

The ACVO credentialing process has two stages: (1) application to the ACVO Credentials Committee to become an ACVO-approved resident, which is required at least 30 days prior to starting an ACVO-approved residency, and (2) application to the ACVO Credentials Committee to take the Certifying Examination, which is due January 15 of the year in which the Examination will be taken. 

All forms and supporting materials for both stages of the ACVO credentialing process must be submitted by email to the ACVO Office at and any fees must be paid by credit card using the secure online website by the appropriate deadline.

For both stages of the ACVO credentialing process, forms or attachments received after the deadline, incomplete forms, submissions not using original and current versions of all forms, and failure to pay all fees will render the application incomplete. 

Applicants should confirm receipt of materials by the ACVO Office; having sent an email does not guarantee that it was received.  Applicants are strongly advised to send all materials well before the deadline to avoid a delay in the start of their residency, in the first stage of the ACVO credentialing process, or a year’s delay in their ability to take the ACVO Certifying Examination, in the second stage of the ACVO credentialing process. 

STAGE 1. APPLICATION TO BECOME AN ACVO-APPROVED RESIDENT: 

  • There is one form involved in the first stage of the ACVO credentialing process:  the Provisional Resident Application.

  • Prior to submission of this form, a Residency Training Program Application must have been submitted to and approved by the ACVO Residency Committee by the provisional resident’s prospective supervising diplomate(s).

  • Once the Residency Training Program Application has been approved, a Provisional Resident Application must be submitted by all provisional residents wishing to become ACVO-approved residents. Time spent in a residency program will not count toward the minimum 36-month requirement until the provisional resident has been accepted by the ACVO Credentials Committee as an ACVO-approved resident.

  • All forms and supporting materials must be sent as pdf documents attached to an email to the ACVO Office at .
  • The required fee must be paid by credit card using the secure online website:

  • The deadline for receipt of all forms and fees is at least 30 days prior to beginning the proposed residency program.

 

STAGE 2. APPLICATION TO TAKE THE ACVO CERTIFYING EXAMINATION: 

  • There are two forms involved in the second stage of the ACVO credentialing process:  The Certifying Examination Application and the Supervising Diplomate Form.
  • The Certifying Examination Application must be completed by all examination applicants every year they apply to take any part(s) of the ACVO Certifying Examination. 
  • The Supervising Diplomate Form must be completed by all supervising diplomates of examination applicants who are submitting their credentials for the first time since 2007. Supervising Diplomate Forms are NOT required for examination applicants who successfully completed the credentials process in 2008 or after, and who are reapplying to take the ACVO Certifying Examination.
  • The Supervising Diplomate Form must also be completed by any supervising diplomate whose supervisory role ends prematurely (due to the resident or supervising diplomate leaving the training program). In these instances, the form shall be completed within 30 days of the severance occurring.
  • All forms and supporting materials must be sent as pdf documents attached to an email to the ACVO Office at .

  • The deadline for submission of the Certifying Examination Application, Supervising Diplomate Forms, and any attachments is midnight, Mountain Time, January 15th, 2013.  Please note that beginning in 2013, the application deadline will be the same every year – January 15th – regardless of which day of the week upon which it falls.

 

Each of the three Credentials forms is divided into the same four sections: (1) DEFINITIONS (repeated on each form), (2) GUIDELINES (instructions for completing the form, different for each form), (3) APPEAL OF ADVERSE DECISIONS (summary of appeal process, repeated on each form), and (4) the body of the form itself. 

Please note that all three forms involved in both stages of the ACVO credentialing process must be submitted via email; a certified copy of the form itself is not required, for any of the three credentialing forms.  Across all forms in both stages of the ACVO credentialing process, the only requirement for a certified copy is on the Provisional Resident Application, Question #13, which requires verification of the veterinary degree. The applicant may choose one of four means of verifying the degree, two of which do not require a certified copy (i.e., North American veterinary license verifiable online, and veterinary school transcript), and two of which do require a certified copy (i.e., of a North American veterinary license, or of a veterinary diploma).

EXAMINATION INFORMATION

Instructions for the ACVO certifying examination have been provided below. Completion and submission of the "Examination Registration Form" with accompanying payment, due by the deadline posted below, is required. Questions concerning the ACVO certifying examination should be directed to the Exam Committee Chair, Dr. Ian Herring at email.
